Macedon Ranges Health Home Care Services delivers comprehensive, professional nursing and healthcare to people where they live. Whether that is in their homes, residential care facilities, nursing homes, schools or even in our community health centre, Macedon Ranges Health believes that we have a role in maintaining and improving our clients' quality of life and independence by providing the care that meets their individual needs.
Our services are provided irrespective of age, race, religion, presenting illness or social and economic status. The only criteria we have are that a person has a genuine need for care and that their needs are able to be met by the services we can provide.
Community Nursing
Most nursing care can be provided in home including care which has traditionally only been available in the hospital environment. Our nurses liaise with the client, their family and other health providers to develop a comprehensive care plan which will ensure that the individual’s health and care needs are met.

All new clients that we meet through our Nursing Service will have a full Health Assessment to ensure that they are getting the care you need to live a full and independent life. We see the assessment as an opportunity for clients to discuss any health issues of concern to them in the safety and security of their own home. Our nurses can assist with issues such as incontinence, diabetic/dietary advice, hearing, dental, eyesight, physical limitations, social wellbeing, transport, cleaning and general health issues. The nurse will review any medications to ensure correct compliance and storage and put any needed strategies in place to assist in promoting compliance.
We can also refer to any of the other health services available at Macedon Ranges Health. These include podiatry, physiotherapy, diabetes education, counseling, volunteer support and activity programs. Many of these services are available with a subsidised fee for eligible people.
Home visits also provide the chance to check the safety of wet areas, kitchens, steps, stairs, floor safety and access to outside of the building.
